Can you help me guess the name of a book I read a few years ago ?

It’s about a homeless H and h, who works in the medical profession. They met when they were runaway teenagers living on the streets. They later become lovers, and when they decide to live properly, the h stands up the H because he feels she is better of without him.

They meet later as adults and she takes home off the streets

Anyone have a guess?
